Make sure that you are warming up really good. If you are using a barbell, try using the db's for a while. Control the movement and get a good squeeze at the top. Make sure that you don't let the db's down too far, as this tends to put alot of stress on the shoulders. Atleast for me anyway.

Is it just flat bench pain? Any other issues with it during other w/o's?

This sounds a bit obvious, but go see a doc.. go get an MRI. Better have him tell you to take several weeks off than several months when something snaps.

I get my physical every so often and I force him to X-ray any twinge at all.

I think i know what youre talking about and Ive gotten it from time to time. And when I did I definitely didnt try and go heavier on that day or heavy at all.

Make sure your stretching. Or maybe you need a day off cause you hit the weights real hard yesterday, especially if you did shoulders the day before.

But when all said and done, if you think you should see a doc. go and see a doc

stop benching for now (for a few weeks) before you make it worse.

if it is truly hindering your workout, and hurting, then see a dr.

warmup of the rotator cuff before presses is extremely important.
also, strenghtening of the rotator cuff in general will only improve all of your compounds lifts.
there are several threads on the Forvm on this subject,
or Google and you'll find lots of exercises out there.

be careful with your shoulders, they are easy to injure and hard to fix.
