I don't think I have ever had a plain hamburger...sounds boring. Unsure the quality of meat at chain places but growing up Carls Jr had a double western bacon cheese burger that was good for the cost as well as a burger place in Newport Beach called TK burger. The bar I manage now has a good flat top grill burger. Meat is 80/20 ( wish it was 93/7) good quality bun and the fixins are fresh. Never hear any complaints. I think a lot of the time it is the flavor of the actual grill that makes a good burger.
